Cards & Services
All of the LifeTrack Cards come with a Microsoft Tag on the back. Each tag is unique to the card holder. Allowing emergency responders to access your emergency information via a smart phone app.
In the event that you have an emergency and the medical personnel do not have access to a smart phone and can not plug your card directly into a USB port, each card has information printed on the back about our LT911 website, this site allows emergency and medical personnel to access your emergency information via a secure web portal.
LifeTrack also has an emergency phone number printed on their card so that responders can contact LifeTrack at any time to get help in accessing your emergency information.

In Case of Emergency Card (ICE Plus):
The LifeTrack Ice Plus Card is an Emergency Medical Identification Card (EMID) and an In Case of Emergency record that can be viewed by emergency personnel. You can upgrade your card anytime to the LifeTrack PHR or LifeTrack Complete product. The In Case of Emergency record has your physicians name and phone number, two emergency contacts, a list of your allergies, your current medications and any special needs you may have. The LifeTrack ICE Plus product is an excellent product for anyone who travels, including students who participate on sports teams or college students who live away from home.
Personal Health Record (PHR):
The LifeTrack Personal Health Record includes the compilation, scanning and encryption of your Personal Health Record. This record can be used in emergency situations and in routine visits to your medical practitioners. Unlike other products that store your medical information, with LifeTrack PHR one of our specially trained nurses assist you in the compilation of your health record. We help you all the way through the process. We will contact your primary care physician and arrange to have your documents sent to us for scanning and inclusion on your Emergency Medical Identification Card.
How the process works ... Once you place your order, one of LifeTrack's Personal Data Specialists will collect your information and will send you a LifeTrack welcome packet. After you receive the packet a call will be arranged for you to speak with one of the LifeTrack nurses. If you have a computer and broadband connection you can go ahead and download and install the LifeTrack PHR software. The welcome packet will explain what information you will need to have ready when the LifeTrack nurse contacts you. On the call one of our nurses will go through a detailed medical history with you. If you have a computer, they will assist you in entering the information into LifeTrack software. If you do not have a computer, they will enter the information in and send you your Emergency Medical Identification Card, with the information stored on it.
What you get ... Once you go through the LifeTrack PHR process you will receive your Emergency Medical Identification Card and a printable version of your medical history. Depending on the complexity of your Personal Health Record, it could take up to 60 days for your doctors to send us your records so we can complete your PHR. If you opt for the LifeTrack Plus or Premium subscription service, you will have access to LifeTrack nurses to assist you with making additions and changes to your PHR.
Complete (PHR & Disaster):
LifeTrack Complete is our most comprehensive package. LifeTrack Complete includes all the benefits of the Lifetrack PHR, but it also allows you to store additional information that is critical to your life and now includes the Living Smart Software. One of LifeTrack's Personal Data Specialists will provide you with a detailed list of what you might want to include in your personal data vault. You will select which information you want to include. If you have the Premium subscription service LifeTrack will assist you in digitizing the data and help you place it on your Emergency Medical Identification Card. Your data is safely backed up and the encrypted data is stored on LifeTrack's secure server.
Fire or Other Disaster Bring your life back to normal faster by having all of your personal belongings documented with photos and lists including pertinent information. You will also be able to store your insurance policies making them available quickly after a disaster.
Medical Emergency Have your important medical information available to emergency medical personnel. With the LifeTrack Emergency Medical Identification card your pertinent information is immediately available to emergency personnel.
Lost Child Have a current picture and vital information about your child with you at all times. If the unthinkable happens, precious minutes can mean the difference between life and death.
Lost Pet With the LifeTrack Complete package we record pertinent information about your pets that can be helpful if they are lost.

Basic:
LifeTrack's Basic service package With this you receive up to 1 update per quarter (4 per year), you receive the Doctor's Corner Newsletter and you receive the data backup service that backs up all of the data in your LifeTrack software.
If you are a LifeTrack Rep this fee will also give you your personalized website and your back office site which will track all of your sales for you.
Plus:
LifeTrack’s Plus Service Package Whit this you receive all of the benefits of the Basic Monthly Service Package, including the Doctor's Corner Newsletter, the backup services for the data stored in your LifeTrack software, AND you receive the LifeTrack Discount Program with discounts on prescription and over the counter drugs, eyeglasses, hearing aids, and many other products. You ALSO receive up to 12 updates per year (one per month) on your PHR or Complete card.
If you are a LifeTrack Rep this fee will also give you your personalized website and your back office site which will track all of your sales for you.
Premium:
LifeTrack’s Premium Service Package With this you receive all of the benefits in the Basic and Plus packages, and ALSO receive the LifeTrack Concierge service and the LifeTrack Personal Computer Backup services. With the LifeTrack Concierge service, if you need any of the documents stored on your LifeTrack card, we will deliver them to you. If you are travelling oversees and lose your wallet, you can contact LifeTrack and if you have scanned an image of your Passport onto the card, LifeTrack will deliver the scanned image to you. If you arrive at a specialist and they don't have your medical records, if they have been placed in the LifeTrack software, we will deliver them to you. With the LifeTrack Personal Computer Backup service, LifeTrack allows you to back up your personal computer to LifeTrack's secure servers, helping you to avoid the costly loss of data if your hard drive fails.
